Churches of Christ Care’s Home and Community Care team provides a range of care and services to clients wishing to remain living independently in their own homes, or other community based accommodation, for as long as possible. Services include domestic assistance, personal care, nursing and allied health care, home maintenance, social and lifestyle services, carer support and respite services. Operating more than 50 home care programs throughout the state in locations from Tweed Heads to Townsville and west to Blackall, the organisation also operates a number of Home Assist Secure programs and Home and Community Care services throughout the state. Services range from comprehensive home care packages delivered under the Consumer Directed Care approach, to low level support programs and healthy ageing programs, and include nursing, personal care, respite, social support services, home maintenance, domestic assistance and transport assistance. Many of Churches of Christ Care’s at-home and community based services are subsidised by state and federal governments. All services operate under, and comply with, the relevant quality standards, regulations and funding body guidelines that govern contemporary home care and community services. Churches of Christ Care operates five Home Assist Secure programs across southern Queensland. Home Assist Secure programs aim to improve the safety, independence and quality of life of people 60 years and over, or people of any age with a disability, who wish to remain living in their own homes. Programs provide information, referral and in some cases, depending on eligibility and need, subsidised assistance with essential home maintenance, security and minor modifications. Home Assist Secure assists older people and people with disabilities to make more informed decisions about home maintenance, repairs, modifications and home security; feel safer living in their home; gain increased physical access within their home; have increased confidence about employing tradespeople for housing related work; obtain help with general and security related maintenance and repairs; and be more able to remain living in their home and community. Churches of Christ Care operates four Home and Community Care programs across southern Queensland. Home and Community Care provides minor home maintenance and modifications for people 60 years and over, or people of any age with a disability, to optimise their capacity to live independently in the community with dignity. The organisation’s four home maintenance services, located in the western suburbs of Brisbane, Redcliffe, Southern Downs and central west Queensland, provide assistance with the maintenance and repair of a client’s home, garden or yard to keep their property in a safe and habitable condition. Home maintenance services include minor maintenance, such as changing light bulbs; minor dwelling repairs, such as changing tap washers; and making safe any damage that occurs within the home that does not require the skills of specialised tradespeople. Churches of Christ Care also provides a home modification service in the western suburbs of Brisbane. Home modification services are provided following an assessment by an occupational therapist and may include the installation of grab rails, ramps, shower rails, appropriate tap sets, safety and mobility aids, and other minor renovations. The Home and Community Care program is a joint Commonwealth, State and Territory initiative.
