Are you or a loved one struggling with medication management?
Mixing up your medicines can be dangerous. Each year more than 140,000 Australians go to hospital with problems caused by their medicines.
Remembering to take one vitamin every day can prove quite a challenge so when you start taking more medication, each of which has a different complicated name, different shapes and colours, some you take in the morning, others at night, some only once a week it is easy to become overwhelmed with managing your medicines.
If you, your parents or your grandparents are struggling with their medication signing up for a HMR (Home Medication Review) might just save their lives.
Our pharmacist Clint Flanigan can do Home Medication Reviews. He can review what medications have been prescribed as well as any over the counter medication you use reguarly to determine the easiest way of managing your medicine. A Pateint Medication Profile is then created which will list not only each medicine but help you understand it, what it looks like, the active ingredient, dose, brands, what other products can interact with it, what it is for and why you take it.
It is also useful to have on the fridge to show your doctor or health care professional exactly what you are taking but also in case the ambulance are called or you are admitted to hospital - the people taking care of you will know exactly what you take.
A Home Medicine Review is a free service and is particularly useful for people who take more than five medicine or have recently been in hospital.
