The Attendant Care Program (ACP) provides portable, flexible and individualised support for people with a physical or neurological disability and/or who need personal help to complete activities of daily living within the community of their choice. Diverse Community Care works with you and others you would like involved to design supports to meet your lifestyle. This is detailed in your care plan, which is reviewed quarterly or as required. To meet your needs Diverse Community Care ensures all services are covered by skilled and suitably qualified care workers. All care workers undergo mandatory checks prior to employment and we encourage you to be involved in the recruitment process so we get the right person for you and your lifestyle. Regular training is provided to our team of care workers to ensure they have the skills to provide you with the best possible service at all times. Services are monitored regularly to make sure you are satisfied with what is in place and there is staff compliance with your agreed care plan outcomes.
