COMMUNITY AGED CARE CASE MANAGER “My parent needs help but refuses to consider a Nursing home” Ageing Parents? Do you have an ageing family member or friend? Do you need help caring for this person? Do you know what alternatives are available? You may need the services of a Community Aged Care Case Manager. As a Community Aged Care Case Manager I organize, resource and monitor delivery of care services and supports. I provide a single point of contact for my older clients and their families. The older person may require simple and/or complex range of services as well as intensive levels of support on either an ongoing, short-term or an episodic basis. I specialize in helping families navigate and access government programs, community services and/or private services to meet the older person's short and long term care need. After carefully assessing each situation, I identify and facilitate the implementation of a personalized care plan and provide ongoing support for each aspect of care.
